My office is conveniently located at 972 Montauk Highway, Bayport, New York 11705.   There is a

private parking lot with adequate parking spaces.

Fees:

My sessions are approximately 55 minutes, and the fee is based on a sliding scale. 

 

I am a Medicare provider accepting new clients at this time. 

 

For other insurances, I am an out-of-network provider, which means that I will supply you with the documentation that will include the service code, my license number, my provider number, fees paid and the dates of service.  Payment is due at the time of the visit. You can then submit this to your insurance company for possible reimbursement.
